NORCAL MUSLIM BASKETBALL LEAGUE — ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY

1. Assumption of risk. I understand that participation in basketball involves inherent risks of physical injury, including sprains, fractures, concussion, heat illness, permanent disability and death. I knowingly and voluntarily assume all such risks, whether known or unknown.

2. Release. In consideration of being permitted to participate, I release and hold harmless the NorCal Muslim Basketball League, its organizers, officers, volunteers, referees, sponsors, and the owners and operators of any facility used by the league from any and all claims arising out of my participation, except to the extent caused by gross negligence or willful misconduct.

3. Medical. I confirm I am physically fit to participate and have no medical condition that would make participation unsafe. I authorize the league to obtain emergency medical treatment on my behalf if I am unable to consent, and I am responsible for all resulting costs. The league does not provide medical insurance.

4. Media release. I grant the league permission to use photographs, video and game footage of me for league promotion, social media, highlight reels and sponsor materials without compensation.

5. Code of conduct. I agree to conduct myself respectfully toward players, referees, staff and facility hosts. The league may suspend or remove a participant for misconduct without refund.

6. Refunds. Registration fees are non-refundable except where the league is unable to provide the ability to play due to circumstances within the league’s control. Once jerseys are ordered, the jersey portion of the fee is non-refundable.

7. Agreement. I have read this document in full, understand it is a release of legal rights, and sign it freely. If registering on behalf of other players, I confirm I am authorized to accept these terms for each player listed.
